You can use form templates to ask for feedback via surveys, sign-up sheets, registration information, and more. You can track anonymous responses using form templates or you can include a field where users can type in their identifying details.
If you're deciding between reactive or template-driven forms, the better choice depends upon what your needs are. Template-driven forms have basic requirements that make it easy for you to add to while being less scalable. Reactive forms are more robust and more scalable. While reactive forms are more complex to use, you can reuse the same form once you've customized it to your liking.

Consent forms are to be included for review with submissions through COUHES Connect. Investigators must provide a copy of the approved consent form to prospective subjects. Use the form as part of explanations/discussions of study procedures and potential risks. Obtain signatures, confirming willingness to participate and to disclose protected health information. The consent form and the authorization form should be executed at the same time. Keep signed forms in your project files in accordance with COUHES record keeping policies.

The forms below are no longer required for submissions through COUHES Connect because the information will be collected within COUHES Connect instead of via the forms below. Investigator should not use these forms unless specified by COUHES. All initial study applications, amendments, or continuing reviews (renewals) must be submitted through COUHES Connect.
The Department of Defense (DoD) Forms Management Program is administered by the Directives Division (DD), Executive Services Directorate, Washington Headquarters Services. In coordination with the DoD, CIO, the DD establishes policies and procedures for the DoD Forms Management Program, including responsibilities for reviewing and approving form requirements. DoD forms satisfy a valid need; essential to accomplish a mission and necessary for the efficient and economical operation of the DoD.
Furthermore, the DoD Forms Management Program serves as the single source of information for blank forms belonging to and used within the Department of Defense. This means that we are responsible for making sure that all Military Services and DoD components remain compliant with established form policies, and that all approved forms are current and readily accessible. We do not receive or process completed forms. For status of a completed form, please contact the appropriate component and/or the Office of Primary Responsibility (OPR) in order to address your concerns.
The FDP Subaward templates included below were designed to expedite and streamline federal subaward review among FDP member institutions. However, non-FDP members institutions may also use the templates. The templates are considered an ongoing FDP demonstration. Workgroups monitor federal developments and make necessary updates (see our committee page for more information). The templates are not meant to be edited.Please keep in mind, however, that the FDP makes no representation or warranties regarding the suitability of these templates for use on any federal or non-federal sponsored projects. The pass-through entity (PTE) is responsible for ensuring all required terms and conditions flow down to a subrecipient. All users utilize these templates at their own risk.The most recent version of the Research Terms & Conditions (RTC), which apply to research awards from a number of federal funding agencies, were approved in 2017. The RTCs are incorporated into the FDP Subaward templates and are also hosted by the NSF for all federal participating agencies here: All templates, samples, tools, and resources are reviewed and updated, as needed, through the efforts of various working groups. Revised versions will be posted to this page in September, or as needed to address changes in Federal requirements. You can also add these details in the issue template with title, labels, or assignees in a YAML frontmatter format.

With issue forms, you can create issue templates that have customizable web form fields. You can encourage contributors to include specific, structured information by using issue forms in your repository. Issue forms are written in YAML using the GitHub form schema. Note: If you used the legacy workflow to manually create an issue_template.md file in the .github folder and enable blank issues in your config.yml file, the template in issue_template.md will be used when people chose to open a blank issue. If you disable blank issues, the template will never be used.
